Note that there are some themes/templates out there that direct the browser to look for the favicon in a different folder than the public_html or htdocs directory (whatever the publicly viewable 'root' is). So either change that bit of code or just add another copy of the favicon to the specified directory.
The code goes between the 'head' tags.
Also note that when monogodo says root directory, he's talking about the public root, not your server root. (If the domain is a subdomain or add-on domain, it will be something like public_html/add-on-domain-name
